Customer Request Type form missing

Hi @Stefania Tarantino

Thanks for your post.  This appears to be a deprecated form from some time ago.  This existed before the request catalog items were introduced as a way for a customer to say set if it was an incident or request.  As Request Catalog Items already determine the request type, there is no requirement to use the Customer Request Type form.  I've removed this from the documentation.
