One of the question is the Service impacted so, this is my assessment: But on the ticket, there are more options for answers: How do I remove the < 1hr, 1-3 hr and 3hrs + options, as i don't need them. If the options < 1hr, 1-3 hr and 3hrs + were among the possible answers at the time the assessment was done, they will still appear in the assessment, to show which answers were given at that time. The assessment doesn't have the first 3 options for hrs, so I am not sure where they come from, or how I can remove them from the assessment question. Have you done any other operation before creating the new assessment, like deleting or updating an old one? I run a test, created a new assessment, added to the BPM then logged a ticket, and all is working as expected. I'm unable to replicate the issue, so any extra info would be helpful to understand what happened. The possible answers are stored in the table : h_sm_assessment_answers so the extra answers should be stored there and need to be deleted. I will delete the assessment and create another one to see if they don't come-up.
