Requests 'losing' BPM: "The business process workflow for this request is no longer available"

I am starting to see these banners appear on some Requests. Obviously that means those requests will become orphaned and no longer perform their allocated function. Found one that was logged last year (maybe fair enough) but now also one raised only 6th October that has no workflow.

I have reported to Support but thought best to also alert here in case this is more widespread.
