enhancement Minor enhancement: more information trapped about errors from "Add User to Organisation" operation

I have a node that adds to what we call a Subscriber List (OG of type General). Some of my testing is giving me an outcome of "failed". But there is no further information available from the node to tell me the reasons for the failure. I suspect these failures are being caused by the fact that the user is already in the group but, like other nodes, it would be great if the error code was trapped and reportable here so that we could put it on the timeline to aid diagnosis. All we have is "outcome".

Even the log gives me nothing:
