@Victor let me know how I can get you more info to help me resolve this.

It could be a data input and saving issue, as the service requests have data entry fields vs incidents which are working fine.
These were created by CustomerSucess team.

Not really, that's all I can see right now. There are no error logs.

We have been experiencing " 'indexOf' of undefined" error when we log service requests. Could this be due to the update?
It started today.
I have also attached an image for the error "failed to log request" and although a new Sr is logged, there is no data captured.
Please could someone shed some light on this.

Updated our ADFS decrypting certficate and updated Hornbill. When saving the profile it error asking for the Entity ID.
Cannot find this on the Wiki.
I tried using this https://service.hornbill.com/<portalname>/lib/saml/auth/simplesaml/module.php/saml/sp/metadata.php/saml but still does not work.
I get the option to pick a profile when login, which is new, then ADFS login page and then I see this error message.
The specified SSO profile [] could not be found

@Victor The service request errors with "Failed to log the request" but the request is logged but the data inputted is blank. I can see the ticket, but when open the page I get "Cannot read property indexOf ' of undefined".
