Custom Buttons - Deletion Confirmation

We have had a number of instances recently where custom buttons have been removed and while investigating we have observed that if someone selects to remove a button, there is no confirmation prompt.

The consequence of this is that someone may inadvertently remove a button which has the potential to disrupt any working practices that rely on it.

Something along the line of 'Are you sure you want to remove this button for all users?' would be good.

It would be even better to have some kind of audit trail to be able to see which user has removed a button but at least a confirmation prompt would make the user realise they were removing a button for everyone, not just themselves.
