Request Card did not move on the Board despite long-established workflow node to Move Card

For the first time in forever (guess the movie...) we've had a Card not move on the (change) Board despite a workflow node to Move Card (which has been there years).

I wonder if anyone else has seen this. I know there have been a bunch of releases recently and perhaps there is an issue. Note that this CR would have moved from the Normal to Awaiting CM Quality Review lane prior to this issue so it's what I would call 'intermittent' or something related to just that stage or node?
