Some users can't complete IC forms

We've had a couple of people complain that they can't complete a form in the live.hornbill portal even though they've filled in all of the mandatory fields. 

Other people using the same form CAN complete the request.

I've also asked a couple of our Service Desk analysts to log the same call by raising the 'Raise New' option in Service Manager, and one of them CAN complete the form, but the other can't, so this doesn't seem to be a problem with 'basic user access' or 'full access' rights?

I'm not sure if this is only affecting one form, but, at the moment, there haven't been any complaints about other forms. 

Could this be linked to the 'Recent Issues' problems?
