Occasionally, the Business Process in the Service Portfolio Cat Item gets blanked-out

On several occasions over the last 6 months, we have had a Request get logged without workflow. It gets reported to me and I find the BPM workflow drop-down is blank. We just have to update it again. Has anyone else found this mysterious behaviour? I know someone is going to say it's a person that's done it but I'm not sure it is. It's one of the reasons I have asked for an audit feature! There's just no way of telling how or who...
