Selecting "Raise New" Incident used to generate a view that looked like this: After making some edits to resolve Board Manager errors, the functionality has changed and now we have this fucntionality. May you help us bring it back to what it was?

Please locate the default progressive capture and ensure it is set to active state. In the example below, my default progressive capture for New Incidents is new incident catalog item flow
