Report an ICT Issue - Form Design Location in Hornbill Administration

Want to alter to look of the following form below. 

This is the form that is presented on the Employee Portal when you click on Report an ICT Issue.

Want to change wording and also add a couple of extra fields for the customer to complete.

You'll need to use a Customised Form to achieve this, and map your Summary and Description to the relevant fields.

This will also allow you to customise the wording and have additional fields on the same form.

The Link describes how to configure the Customised Form, it's parent page gives a wider view of the Progressive Capture Designer where you would configure the Capture.
