This is the form that is presented on the Employee Portal when you click on Report an ICT Issue. Want to change wording and also add a couple of extra fields for the customer to complete. Steve Giller Posted October 22, 2020 Share Posted October 22, 2020 You'll need to use a Customised Form to achieve this, and map your Summary and Description to the relevant fields.
