Unable to log change request via Employee Portal

Hi,

I am unable to log a change request using the Employee Portal.

I fill out the form, all mandatory fields are completed but the next button is greyed out. 

Logging the request via service.hornbill.com or via the analyst view of live.hornbill.com works fine (same PC).

This is impacting only one RI as far as I can tell.

So it have to be tomorrow with API key as I won't be in office tomorrow and now I'm on my way to airport. 

Anyway have few more questions, what type of fields are used, also do you use conditions to change mandatory flags?
