Attachment Upload - Form Properties Issue?

Is there an issue with using Attachment Titles and Info Message in Add Attachment nodes??  I am using this for some of my procaps and notice that  all of a sudden the Attachment Titles and Info Message disappear and all that displays is the upload document button, without the wording around it that I have entered in these fields. This is happening on live procaps so is very worrying!?

An example of a procap where this just happened is our HRA Senior Management Appointment procap.  It was fine this morning but now its all blank again, both in the procap via self service and in the BPM the fields are blank too?

I've just tested on my instance.  So far it is displaying correctly on the Attachment form within Progressive Capture for me.  Has this been reported by multiple users?  Is there a particular Internet Browser that is used when this occurs or does it happen on all browsers?

It is happening to me in Chrome and also IE.  No-one else has reported it to me but as the wording that I entered has disappeared from the node itself in the ProCap I am assuming it will be everyone who cant see it.

I'm going to have to enter the wording back in as this is a live process and it needs to be there.  If it happens again I will let you know.
